Community Case Management
Community Case Management provides social case management services to people living with HIV/AIDS. Legacy’s trained case managers thoroughly assess each patient’s need for health and social services. These needs could range from transportation to food and from housing to child care.
After the assessment, Case Managers work with the patient to develop a service plan. Items on the plan may involve: providing referrals to community, social and medical services; providing information on HIV/AIDS; assisting with completion of applications and other paperwork; advocating for clients to decrease service gaps; and removing language and transportation barriers to accessing care services.
Legacy works with a broad network of community-based organizations, public entities and healthcare providers including:
• WIC, Social Security and METRO offices
• Homeless shelters and transitional living facilities
• Food banks
• Substance abuse treatment facilities
• Support groups
• Mental health counseling centers
• Hospice services
• HIV prevention providers
• Child care services
These services are free to HIV-positive patients who are eligible under Ryan White CARE Act funding.
Medical Case Management
Medical case management works with HIV-positive primary care patients to achieve optimal health by improving treatment adherence. Because of the vital importance of taking HIV medications in a consistent and timely manner, many patients need guidance and support. Medical case managers work individually with each patient, discussing any barriers to medication adherence and providing tips to taking medications daily.
Medical Case Managers work closely with Legacy’s healthcare providers to identify and address obstacles. This is in an effort to enhance patient access to all healthcare related services available in Legacy’s continuum of care. Medical Case Managers also link patients to other healthcare related services including specialists, diagnostic testing, and hospitals.
These services are free to HIV-positive patients who are eligible under Ryan White CARE Act funding.
