Primary Care Providers

Rachna Bharti, M.D., joined Legacy Community Health Services in January of 2014. She earned her Doctor of Medicine degree from Lady Hardinge Medical College and then went to East Tennessee State University for her family medicine residency.

Prior to joining Legacy, Dr. Bharti worked as a Hospitalist in the Texas Medical Center in Houston, Texas. Dr. Bharti is a member of American Academy of Family Physicians, and has received awards for research projects and for giving presentations at Tennessee Academy of Family Physicians.

Dr. Bharti enjoys cooking, hiking, and outdoor activities in her spare time.

Jim Graham, DRPH, RN, FNP-C, NP, joined Legacy in August of 2006. He earned his Bachelor of Science in Nursing from the University of Texas Medical Branch at Galveston, and then went on to receive his Masters in Nursing from Texas Woman’s University. Lastly, he obtained his Doctorate in Public Health from the University Of Texas School Of Public Health.

Prior to joining Legacy, Graham spent 14 years as a Flight Nurse with a local helicopter EMS. He also worked as a Nurse Practitioner in the emergency department and in clinical research while completing his doctorate. His dissertation on trust and HIV health care practices resulted in two published articles and two poster presentations at the 2010 International AIDS Conference in Vienna, Austria.

David J. Klarberg, MSN, FNP-BC, joined Legacy Community Health Services in December of 2013. He earned his Bachelor of Science in Nursing from The University of Texas at Austin in 2008, and then went on to receive his Master of Science in Nursing from The University of Pennsylvania in 2011.

Prior to joining Legacy, Klarberg worked as an associate medical director, clinical research sub-investigator, and nurse practitioner at St. Hope Foundation in Bellaire, Texas. While there, he helped facilitate several clinical trials in the area of HIV pharmacotherapeutic research. In late 2012, Klarberg received a specialty certification from the American Academy of HIV Medicine. His current clinical interests include cardiovascular disease risk reduction, metabolic syndrome management, and new HIV prevention and treatment strategies.

Klarberg enjoys urban cycling, and is an avid fan of sushi. He has played electric and bass guitar for over 15 years and has recently taken an interest in music production and recording techniques. He is a lifelong fan of the Star Trek television and film series, and his home office is adorned with memorabilia from the franchise.

Mark Levine, M.D., joined Legacy Community Health Services in January of 2007. He earned a Bachelor of Arts in History from Dartmouth College, and went on to receive his Doctor of Medicine degree from The University of Texas Health Science Center at Houston. He completed his residency in Family Practice at Middlesex Hospital in Connecticut.

Prior to joining Legacy, Dr. Levine spent several years working around the country in under served rural communities where he provided the full range of primary care in a variety of challenging environments. Dr. Levine is a member of the American Association of Family Practitioners.

Dr. Levine enjoys spending time with his wife and young daughters, from whom he learns more about life than he ever learned in medical school. His favorite thing about working at Legacy is the wide variety of patients he comes into contact with, many of whom challenge and amaze him on a daily basis.

Lourdes Murra, MD, joined Legacy in January of 2012. She earned her Doctor of Medicine degree from Louisiana State University Health Sciences Center in New Orleans.

Prior to joining Legacy, Dr. Murra was a Family Practice Resident at the Ochsner Medical Foundation. She also worked in the Cy-Fair area for Tenet Healthcare. Dr. Murra is a member of the American Medical Association

Dr. Murra enjoys spending her free time with her family.

John J. Nemecek, M.D., joined Legacy Community Health Services in January of 2010. He earned his Bachelor of Science degree from Eastern Illinois University and went on to receive his Doctor of Medicine degree from the Southern Illinois University School of Medicine.

Prior to joining Legacy, Dr. Nemecek worked with a number of other Houston-area HIV Physicians and organizations and served as the Chairman of the Family Practice Department at Twelve Oaks Hospital. Dr. Nemecek is a member of the International AIDS Society and is the Chairman of the Board of Directors at the Houston HIV/AIDS Resource Group.

In his spare time, Dr. Nemecek enjoys his pet Pembroke Welsh Corgi and is active in his church.

Mary Pastor, MD, joined Legacy in October of 2012. She earned both her Bachelor of Science in Biology and her of Doctor of Medicine degree at Cebu Doctors’ University in the Philippines.

Dr. Pastor trained and worked in New York and New Jersey prior to moving to Texas. She enjoyed a successful private practice in South Texas for five years before joining her husband, another busy physician, in the Beaumont area. She has 13 years of FQHC experience, with Legacy being the third such agency she’s joined. Dr. Pastor currently serves as the Health Authority in Hardin County and oversees public health programs for both Orange and Hardin Counties. Working for Legacy reinforces her commitment to public health, community awareness, and meeting the health care needs of the uninsured, underinsured, and displaced populations.

Dr. Pastor enjoys boating and deep sea fishing. Both she and her husband are kept busy at home with a menagerie of dogs of all breeds and sizes, all of whom were rescued or adopted. Her lifelong dream is to build a dog park and shelter in Southeast Texas.

Natalie Vanek, M.D., joined Legacy Community Health Services in July of 2007. She earned her undergraduate degree from Baylor University, and then went on to receive her Doctor of Medicine degree from The University of Texas Health Science Center at Houston. She stayed there for her internship and residency in Internal Medicine. Dr. Vanek specializes in infectious diseases, with a special interest in HIV care for adolescents and adults.

Prior to joining Legacy, Dr. Vanek served on the faculties of both The University of Texas and Baylor College of Medicine in their Infectious Diseases departments. She has worked at Thomas Street Clinic, providing care for HIV positive Houstonians and participating in clinical trials.

Dr. Vanek is now the vice-chair of the Texas HIV Medication Advisory Committee, which she has served on since 2000. She received H Magazine¹s Houston¹s Top Doctors award for Infectious Diseases in 2011 and 2012. Dr. Vanek has published articles in Infection & Immunology, Clinical Infectious Diseases, and Medical and Pediatric Oncology. She has been invited as a guest speaker at numerous conferences.

Dr. Vanek enjoys hanging out with her two teenage sons, cooking, and going to the beach in her spare time.
