Award Recognizes Innovation in Pharmacy Services, Medication Access, and Patient Health Outcomes
HOUSTON (August 28, 2026) – Helping patients access medications, manage chronic conditions, and achieve better health outcomes has earned the Legacy Community Health Pharmacy team the 2026 Excellence in Patient Outcomes Award from the Texas Pharmacy Association (TPA). The award recognizes organizations that advance pharmacy care and improve patient health outcomes through innovative, patient-centered services and was presented during the 2026 TPA Conference & Expo.
Legacy earned the recognition for expanding access to pharmacy care, reducing barriers to treatment, and empowering patients to take a more active role in managing their health. Through medication counseling, chronic disease management, specialty pharmacy services, and patient support programs, Legacy pharmacists help improve health outcomes for patients throughout Houston, Fort Bend County, Beaumont, and communities across Southeast Texas.
Pharmacists play a pivotal role in helping patients manage chronic conditions and remain adherent with their treatment regimens. At Legacy, pharmacists are integral members of the care team, working alongside providers and other healthcare professionals to deliver coordinated, patient-centered care and support continuity of care across the communities we serve. The Legacy Pharmacy team actively reduces barriers to care through free prescription delivery, statewide shipping, multilingual prescription labeling, and affordable medication programs.
Improving Access to Medications and Care
Beyond traditional dispensing services, Legacy pharmacists provide clinical support, medication counseling, and pharmacist-led services that help patients understand and remain on track with their treatment plans.

- Statewide prescription shipping
- Medication labels available in more than 120 languages
- Voice-enabled technology for patients with visual impairments
- Affordable medication programs through the federal 340B program

Expanding Pharmacy Services Across Southeast Texas
The award comes as Legacy continues to expand pharmacy services across Southeast Texas, including new locations in Pasadena and Acres Homes that bring pharmacy care closer to patients and families. These efforts help bring affordable medications, coordinated care, and pharmacy expertise closer to the patients who need them most.
Legacy is also investing in the future of pharmacy through its Pharmacy Internship Program. Students from the University of Houston, Texas Southern University, and other institutions gain hands-on experience in patient care, immunizations, health screenings, community outreach, and quality improvement projects.
