Legacy has recently updated its intake and consent forms and need for all patients to complete them at the time of their next visit. Once completed, these forms will be good for a year, but we wanted to let you know you will be asked to complete it. Due to the increase in COVID-19 cases, we are experiencing a higher than normal call volume. We are working hard to answer calls and respond to messages. Thank you for your patience.
If you are calling for COVID-19 test results, please allow 7-10 business days for results to be returned. You will be called when your test results are received from the lab. You may also access up-to-date information on your lab test results by visiting the LabCorp PatientTM portal or downloading the app on your mobile device.

No Show Policy
Missed medical appointments hurt us all!
You must call ahead if you’re going to miss an appointment.
When a patient misses an appointment or cancels at the last minute, we are unable to offer that time slot to another patient.PLEASE SEE OUR NO SHOW POLICY
Legacy patients have access to health center staff after business hours. For non-emergency health concerns/questions, please call (832) 548-5000 to be directed to a Registered Nurse who will assess concern/question and determine whether a Legacy physician, dentist, or behavioral health specialist should be consulted. The nurse is also able to offer clinical advice and guidance. For emergency situations, please call 911 or go to the nearest emergency room.
