Legacy Community Health knows nothing is more important to you than your health, and we take that responsibility seriously. Our qualified staff will continue to provide you with the same high standards of care you have come to expect.

We have a process to transition your care and all upcoming appointments to another Legacy clinician. Legacy staff are available to assist you in the transition of your care and can be reached directly at (832) 548-5000.

Your medical records are confidential and will remain at Legacy for use by future Legacy clinicians treating you. If you elect to receive care outside of Legacy, please visit our website at LegacyCommunityHealth.org/Forms to obtain a Release of Information form so your medical records can be available to non-Legacy clinicians.
