New Patient Appointment –
Office Visit –
MyChart Video Visit – EST – Video visits for established patients
Telemed – MyChart – New – Video visits for new patients


When selecting Insurance Information please note the following if you select “No Insurance”:

Patient Pays – Select this option if the patient, themselves, is financially responsible for the visit. The patient may wish to separately schedule an appointment with our Eligibility department to find a path to making the appointment more affordable. Learn more here.

Someone Else Pays – Select this option if a parent or guardian will be financially responsible for the visit.


Tipo de cita

Niño sano (recién nacido – 11 años) (Well Child) – incluye atención preventiva rutinaria, inmunizaciones / vacunaciones
Niño Sano Extendido (12 – 18 Años) (Well Child Extended) – incluye cuidado preventivo rutinario, inmunizaciones / vacunas
Cita con el nuevo paciente (visitas de niño enfermo) (New Patient Appointment) – evaluación de los síntomas
Telemed – MyChart – Nuevo (Telehealth) – Video visitas para nuevos pacientes

Si sólo necesita inmunizaciones/vacunas o resultados de laboratorio sin una visita de niño sano, por favor llámenos al (832) 548 5000.
