Healthcare Providers and Care Team

Will my healthcare provider and/or care team change?

On July 13, 2022, Jordan Family Healthcare will officially become part of Legacy.

What changes can I expect?

You will continue to receive personalized, quality care and communication with your Jordan Family Healthcare team, just as you always have. What will change is the added benefit of access to Legacy’s comprehensive range of health and wellness services and programs (read more under “Comprehensive Services”).

What happens to my medical records?

Your medical records are confidential and will remain with us for use by Jordan Family Healthcare and Legacy providers treating you. If you elect to receive care outside of our practice, you may request your medical records by visiting our the patient portal on our website at drjordanmd.com or calling us at (832) 834 3800.

Insurance & Flexible Payment Options

What payment options will be accepted?

We will continue to accept most health insurance plans, including HMO/PPOs, Medicare, Medicaid and CHIP. Legacy’s Eligibility Specialists are available to discuss questions about your insurance plan and payment options, including Affordable Care Act Marketplace, Medicare, sliding scale fees, and ways to make care more affordable.

What if I receive a letter from my insurance company stating that Dr. Jordan or my other providers will no longer be covered? What should I do?

Call us! We have a team in place to answer all your questions and help with any insurance and billing needs.

 

Scheduling Appointments & After Office Hours Support

How do I schedule, reschedule, or cancel an appointment?

Our contact information remains the same, so appointment scheduling, rescheduling and cancelling appointments will continue to be handled by calling (832) 834 3800 or by using the online scheduling portal www.drjordanmd.com/schedule-appointment.

Who will respond to my calls and questions after office hours?

A third-party team is providing a registered nurse after office hours to address your questions. Dr. Jordan and team will remain a back-up for this after-hours service.

Why are you making this change?

Joining with Legacy allows Jordan Family Healthcare to continue to provide the same care and service we have always provided while also becoming part of a network of clinics across Southeast Texas. This means we are able to offer even more comprehensive services to our patients, including Behavioral Health, Dental, Endocrinology, Infectious Disease, LGBTQ+ Services, OB/GYN, Pharmacy, Vision, and the Southwest Pediatric Walk-in Clinic, which is open to all children seven days a week.

 

Prescriptions and Legacy Pharmacy

Will I continue to get my prescriptions filled at the same pharmacy?

Legacy Pharmacy will fill and deliver your prescriptions and refills to your home. Legacy Pharmacy offers a full range of services for all patients, including immunizations, health coaching and education, medication syncing, medication therapy management (asthma/COPD, diabetes, hypertension, hyperlipidemia), and one-on-one medication counseling. You may also elect to continue to get your prescriptions filled at the pharmacy you are currently using.

What makes Legacy Pharmacy different?

Legacy Pharmacy’s specially trained pharmacists work to quickly address a member’s medication needs, help coordinate care, provide education, and improve access to healthcare between healthcare visits. They also believe health is personal and should be treated that way.
