Know Before You Go
Legacy has recently updated its intake and consent forms and need for all patients to complete them at the time of their next visit. Once completed, these forms will be good for a year, but we wanted to let you know you will be asked to complete it. You may also download, complete, and bring the forms with you to your next appointment.

This transition will bring you added benefits like on-site check-in kiosks and live wait times; plus, all your existing lab results will remain accessible via the Legacy Patient Portal. Future lab results can be accessed through both the Legacy Patient Portal and the Quest Diagnostics MyQuest Portal.
My Legacy patient portal is a website that allows you to interact and communicate with your health care team via the Internet. The portal services are available to you at anytime and allow you to acces your medical records along with a variety of other functions.
Your Health Information Is Private, Secure, and Protected
To ensure that your private health information is safe from unauthorized access, the My Legacy portal is hosted on a secure connection. The application is accessed via an encrypted, password-protected login that only you have access to. Legacy staff cannot view this information, and therefore cannot accidentally provide your username and password to unauthorized people. Your account cannot be accessed without the password, nor can changes be made to your account profile.
You will need to request your personal PIN from your clinic in order to create an account.
