Legacy Community Health wants a health care system where every patient’s basic needs are met as part of good care. Our Health Advocates check if you have enough food, housing  transportation and financial help. They then connect you with community resources so you can be healthier and have a better quality of life. We offer programs and support to help you reach your best health outcomes.

Social Drivers of Health (SDOH) Coordination

Legacy Community Health is committed to helping and supporting all of our patients’ needs, including assistance with transportation to and from medical appointments and other non-medical reasons. 

Depending on your individual circumstances, we can assist with coordinating transportation with your insurance provider or other community based resources, and other assistance programs depending on eligibility.

In August 2018, Legacy started checking all primary care patients for food insecurity. This means we ask if people have enough food at home or if they worry about running out of food. Our goals were to: 

  • Make sure every clinic asks every patient about food needs. 
  • Share food resources to patients who need help. 
  • Teach doctors and staff how to ask about these needs and how to help patients get the right support.

Has lack of nutritious food increased your risk of developing a chronic condition like diabetes or hypertension? 

Ask your doctor about FoodRx, a food prescription for fresh produce and other healthy food items. 

The program is a partnership between Legacy and the Houston Food bank to create a unique opportunity for our patients when referred to or by a provider and screened positive for food insecurity to redeem an patient food prescription. Upon redeeming patients are able to redeem their Food Rx twice a month at Food for Change Markets, where they can select up to 30 pounds of free fruits and vegetables plus 4 additional healthy items. Nutrition education material is also available.

Free or Low Cost Activities for Adults and Children 

There are many fun and low cost activities in our area. These activities help adults and children get moving and make new friends. Your doctor can now refer you to community programs that support healthy living. 

What the Program Offers 

  • Free or lowcost classes about exercise, healthy eating, and stress 
  • Walking groups that help families stay active together 
  • Fun group activities that make healthy habits easier 
  • Programs close to home and simple to join 
  • Help and resources for longterm health for all ages 

Why It Matters 

These programs help adults and children stay healthy. Walking groups and classes help people feel supported and part of a community. It is easier to stay healthy when you are not doing it alone.

This program connects patients to information and resources from Houston Food Bank and Harris County Public Health. The goal is to help people get free or low cost health and food benefits, like: 

  • SNAP 
  • TANF 
  • Medicaid, Medicare, and CHIP 
  • WIC 
  • Gold Card 
  • And other help programs 

Community Health Workers educate about eligibility requirements and assist patients and community members to fill out applications for these benefits.
