Legacy is proud to work with Health Choice Network (HCN) in delivering MyChart to our patients. 

Please be aware that when scheduling an appointment online, you will have two options:

Existing patients on Legacy MyChart should select “Log in and schedule”.

New patients / existing patients not on MyChart should select the “Continue as guest.”.

For services not shown below, please call (832) 548 5000 to schedule an appointment.

Patients can get support for MyChart by calling the Legacy Contact Center during normal business hours at (832) 548 5000, emailing us at MyLegacy@legacycommunityhealth.org or in our FAQ.
