Patient Navigation

People with HIV/AIDS often require a complex combination of social and medical resources of over a long term period in order to manage their health. People unfamiliar with these resources may not be aware of the services available to them and may not appreciate the need to access health and social services. Patient Navigation is an effective and efficient means of managing the client’s transitions through medical and social services networks. Clients are assigned to a Patient Navigator to assisting them in the process of entering into medical care and helping to clarify both medical concerns/issues and providing of referrals.
