10 Legacy Community Health Clinics Earn National PCMH Recognition

Designation affirms Legacy’s role in advancing high-quality, community-based care across Southeast Texas

HOUSTON, TX (June 4, 2026) – Ten Legacy Community Health clinics have earned national recognition as Patient-Centered Medical Homes (PCMH), making it easier for patients across Southeast Texas to get coordinated, high-quality care. Awarded by the National Committee for Quality Assurance (NCQA), the recognition is one of the nation’s leading benchmarks for patient-centered, team-based primary care.
