General

Legacy Community Health Services is a full-service, Federally Qualified Community Health Center that provides comprehensive, primary health care services to all Houstonians in a culturally sensitive, judgment-free, and confidential environment. Legacy has specialized in HIV/AIDS testing, education, treatment, and social services since the early 1980s. We also provide care for other chronic health conditions like diabetes and high blood pressure. Generous financial support from individuals, businesses, and charitable foundations allows Legacy to provide no-cost or low-cost health care to nearly 50,000 men, women, and children each year.

Legacy provides an array of services to meet your health care needs, including Primary Healthcare, Pediatrics, OB/GYN, Family Planning, Behavioral Health Services, Vision Services, Dental Care, Vaccinations/Immunizations, HIV Testing and Counseling, STD Testing and Treatment, Wellness Services, Education and Outreach, and Social Services. We also have a pharmacy at our Montrose and Lyons campuses.

Anyone can use our services. We tailor our care to each patient regardless of age, sex, race/ethnicity, sexual orientation, gender identity, or ability to pay.

You can request lab or test results, or view them at any time, using the Legacy Patient Portal.If you have not already registered for the Patient Portal, request a PIN from our staff on your next visit.

In case of an emergency, please call 911.

We support patients having access to their health information. Beyond the Patient Portal and our Pharmacy RX Local App, we also support the sharing of health information via applications that use an Application Programming Interface (API), which many mobile device applications use. While we have enabled the necessary solutions to receive requests from API applications, we are unaware of any apps having been configured to connect to our data. Our website, LegacyCommunityHealth.com, will be updated when we become aware of any applications that our patients can begin to use.

Campus Information

Legacy has five main locations and a variety of secondary sites across the greater Houston area, including Baytown and Beaumont. In each of these locations, you will find high-quality, affordable health care for the entire family in a confidential, judgement-free environment.

Every location has a different set of hours. Please click on your respective location to find its hours.

After clinic hours, on weekends or holidays, Legacy patients may contact Legacy’s on-call answering service, which is staffed by nurses able to assist patients with their concerns and, as necessary, contact a Legacy medical provider on-call with access, via remote connection, to the patient’s records.

We have free parking available onsite at all of our locations. It is advised to arrive early as parking spots can go quickly.

Your First Appointment

We prefer you make an appointment, but walk-ins are accepted on a limited basis.

Please call (832) 548 5000 to make an appointment at one of our locations.

If you need to be seen on the same day, please call (832) 548 5000 and explain your medical situation.We will do our best to fit you in if there is availability. If it is an emergency, please dial 911.

If you have insurance, please bring a picture ID along with your insurance card.
If you do not have insurance, you will need to meet with an Eligibility Specialist. Please check our Eligibility Requirements before coming to your appointment.

Many of our providers and staff are fluent in English and Spanish. We can also provide an interpreter for a limited number of other languages.

Payment

Services are not free, but we do offer a sliding fee scale for those who qualify.

Legacy accepts cash, check and most credit cards.

The inability to pay should not be an obstacle in getting the health care you need. Legacy has many options available to help you through the payment process. Please make an appointment with our Eligibility department by calling 832-548-5000.

Patients must pay at the time of service. We do not send bills out to our patients.

You may specify where you want your money to go when you make a donation.

A receipt will be sent to your email after you submit your online donation.

Volunteer

To get in touch with the Manager of Volunteer Services, please email volunteer@legacycommunityhealth.org or call 832-548-3030. She is able to speak with you in person every Thursday from 10 am to 1 pm. Please make an appointment by calling the number above.

The Volunteer Department does not oversee or register internships/externships. Post-Secondary students are not to be considered volunteers while receiving academic credit. Please contact HR at HR@legacycommunityhealth.org. We also do not offer shadowing or observing medical physicians,volunteers supporting patients, families, or staff through various assignments.

Your level of involvement is up to you. We accept volunteers looking for a couple of hours once a week, those looking to volunteer every other week, or those who want to participate a few times a year at special events.

Legacy has 5 campuses throughout the greater Houston area. Our events are held at many different venues, so we’ll find a spot for you!

Our volunteer coordinator will call to schedule an interview at which you can talk about your interests, how much time you have and other specifics. A background check is then required for all volunteers. A TB test is required for those interested in volunteering at the clinical level. We hold orientation sessions for all new volunteers to provide information, answer questions, and welcome you to the Legacy family!

We take the well-being of our staff members, volunteers, and patients very seriously, so this process has been implemented to ensure a safe environment for all.

TB tests ensure the safety of our patients as well as your own. You will be required to take a TB test only if you volunteer on the clinical side.

Volunteers at one of our campuses must be at least 18 years old. There may be a way to involve younger family members at some of our special events.

To name just a few: You get to work with some of the most dedicated people in Houston. You will have a positive impact on the families we serve. You get an annual TB test and an awesome t-shirt. We could go on, but the bottom line is that you will help us build a healthy community and have a great time doing it.

These may be written by the Manager of Volunteer Services for employment, education, or scholarship references upon completion of a minimum of 40 hours of service.
