If you are a Legacy patient at our Baytown clinic, we want to hear from you. On Saturday, February 15, 2020, Legacy will host its Patient Story Program at our Baytown Clinic, providing patients with FREE family photos, or business head shots, as a “thank you” for sharing your Legacy success story.
At Legacy, we believe in the power of storytelling, and we know your story can help inspire others and make a difference in the work we do. That is why we are bringing this important program to Baytown. Interested patients are encouraged to fill out this story form and submit to it to James Lee, Legacy’s program manager for the Patient Story Program at jlee@legacycommunityhealth.org.
Legacy Baytown’s Photoshoot will take place on Saturday, February 15 from 12:00-5:00pm on the 4th floor of Legacy San Jacinto Clinic at 4301 Garth Rd.
Patient Story Program FAQ
- Who can participate? All Legacy Baytown patients are eligible to participate. Interested patients must email a completed story form to jlee@legacycommunityhealth.org ahead of the scheduled photo shoot.
- What kind of stories are you looking for? Patient success stories can be as simple as receiving annual exams at Legacy, or as extensive as discovering a life-changing diagnosis. The important thing is that you are able to communicate how Legacy has helped you maintain or improve your health.
- How can patients participate? The Legacy Baytown photo shoot is scheduled for Saturday, February 15, 2020. Patients interested in participating must complete a patient story form and schedule an appointment with James Lee, program manager for the Patient Story Program.
