Legacy Acres Homes Brings Quality Care Closer to Home

A diverse group of people stands in front of a modern building with a large "LCH" logo. A sign shows appreciation to Houston Methodist Hospital Foundation. The individuals are smiling and dressed in business casual and formal attire.

Legacy Acres Homes, our newest clinic at 3011 W. Gulf Bank Road, officially celebrated its grand opening on October 2, after serving patients since this summer.

The new location offers primary care, women’s health, behavioral health, and more.

“Acres Homes has spent generations building something worth investing in, and families here deserve care that reflects that,” said Robert Palussek, CEO of Legacy Community Health. “At Legacy, we show up in the communities where care gaps persist, and we stay. This clinic is designed to be a health care home, where individuals and families can access coordinated care and support through every stage of life. It’s a place where a mother, her children, and her parents can all be seen.”

Chinelo Nweke, Vice President of Clinical Operations at Legacy Community Health, said the community has embraced the new clinic.

“This is their clinic, in their community, and I think that sense of ownership and pride has been really special to see,” said Nweke.

Why Did Legacy Choose Acres Homes?

Few Houston neighborhoods have a history as long or as proud as Acres Homes. More than 54,000 people call the area home today.

Acres Homes has also faced real challenges when it comes to getting care. About one in four residents doesn’t have health insurance. Close to 39% of adults have high blood pressure, and nearly 19% have diabetes.

These conditions can be managed well with regular care, but that gets more difficult when the closest clinic is a long drive or several bus stops away. It’s understandable that people put off checkups or wait until something feels urgent. Having a clinic in the neighborhood helps make routine care part of everyday life again.

What Health Beyond Care Means in Acres Homes

Our Health Beyond Care approach comes from a simple belief that good health depends on more than one visit to the doctor. It also depends on having a place you trust, a care team that knows you and services that work together.

That idea guided how the Acres Homes clinic was planned. The 26,200-square-foot building brings many of the services families need into one place, including:

  • Adult medicine
  • Pediatrics and vaccinations
  • OB/GYN care, with free walk-in pregnancy testing available weekdays from 8 a.m. to 4 p.m.
  • Behavioral health
  • An on-site pharmacy opening spring 2027 where Legacy patients can get discounted prescriptions
  • Eligibility specialists who can help with Medicaid, CHIP, and Affordable Care Act applications, as well as sliding scale fees

How Does Having Everything in One Place Help?

When services are connected, it’s easier for your care team to see the full picture of your health. If you come in for a checkup and your provider notices your blood sugar is high, you can get follow-up care in the same place.

If you’ve been feeling anxious or overwhelmed, a referral to behavioral health doesn’t have to mean starting over somewhere new. And when your visit is over, you will be able to pick up your prescriptions before you head home.

This kind of coordination matters for people managing chronic conditions, and it matters for families too. Parents can bring their children in for well visits and vaccinations at the same clinic where they get their own care. Expecting moms can stop in for a walk-in pregnancy test, get prenatal care close to home, and keep coming back after the baby arrives.

Made Possible Through Partnership

The Acres Homes clinic was made possible by a transformational gift of more than $50 million from Houston Methodist, which also funded Legacy’s new clinic in Pasadena. Legacy and Houston Methodist have worked together since 1998 on programs focused on patient care, medical education, and community health.

The clinic is also expected to support 96 full-time jobs, when fully-staffed. That makes it a long-term employer in Acres Homes as well as a place to get care.

Nweke said Legacy has been intentional about not simply opening a clinic in Acres Homes but becoming part of Acres Homes.

“That means listening to the community, hiring from within the community, building relationships with community partners, showing up at community events, and making sure the services we provide reflect what residents need,” she said. “Our team understands that trust isn’t created just because you put a building in a neighborhood. You earn it by consistently showing up and serving well.”

How Do I Make An Appointment?

New patients are welcome, and appointments are available now. Legacy accepts most HMO and PPO plans, Medicaid, CHIP, Medicare, and Medicare Advantage. As the largest Federally Qualified Health Center in Texas, we see patients regardless of their ability to pay.

“When you walk through our doors, our goal is for you to feel welcomed, respected, and cared for,” said Nweke. “We’re here not just when you’re sick, but to be a long-term partner in your health.”
